APHO had made health screening compulsory for all passengers departing from India. Please fill the form and take 2 copies to save time and avoid any inconvenience on departure.
Effective 07th May on all KL flights from India to Amsterdam, passengers will be asked to fill the self-declaration form. All passengers will have retain the Health Screening Form, as same will have to be presented to Dutch officials upon arrival in Amsterdam.
